John Rambo (Sylvester Stallone) is older and returns to the only place he can call home, spending his time training horses and trying to keep his PTSD contained, only to be pulled into an all-out battle by Mexican sex traffickers that kidnap his adoptive niece (Yvette Monreal). – A man leaves a club with a woman, and they go to his pickup truck where he lays her back onto the front seat and begins to unfasten his belt (presumably to have sex with her; please see the Violence/Gore category for more details). A young man leans over toward a young woman attempting to kiss her and she moves away. Men and women in a club scene dance together and a man tries to talk to a young woman and she looks away (please see the Substance Use category for more details).
► A young woman lies on a floor and pulls her skirt down to try to cover her legs (presumably after having been raped). A young woman wears a low-cut tank top that reveals cleavage in a few scenes. A young woman wears short-shorts. A woman wears low-cut tops that reveal cleavage in several scenes.

– An extended sequence shows armed men approaching a house where one is shot with an arrow, another is shot by a bullet (we see splatters of blood) and a few men are doused with gasoline and set on fire (we see them engulfed); the men search through tunnels where one man is stabbed in the chest by a trap, a man’s head is shot off (we see blood, matter and the remaining bloody neck area), two men are shot with incendiary magnesium shards and they burst into flames when struck, a man is run through the head by a long metal spear, a man’s leg is hacked off with a machete (we see the leg separate and blood pours as the man screams and he is then shot), a man falls in a hole in the ground and is struck in the back of the head with a hatchet, two men fall in a pit and land on metal spikes (we see them with several spike wounds and a lot of blood), a grenade blows a couple of men up, a man’s face seems to be blown off, a man is nearly trapped when a tunnel collapses, and a man is shot in the abdomen and the shoulder (we see blood).
► A man is shot with four arrows and pinned to a slat wall where we see blood and he yells; another man stabs him in the chest and draws the blade down to his abdomen, punches into the open wound with a loud splatter and pulls out the man’s heart (we hear a throbbing sound), and throws it on the ground with a splat. An armed man is disarmed by another man, surrounded by several other men, punched in the face and then kicked and punched repeatedly while he is on the ground (we see his very swollen and bruised eyes and face, and a bloody gash on his forehead); one of the attackers says, “Throw him in acid” (they do not), and another man carves a large ‘X’ on his cheek with a knife (we see the blade cut the flesh and hear squishing). A man enters a brothel and beats several other men that he encounters using a hammer (blood sprays and splatters); he hits them in the head and face and one man in the crotch and then the head. A man is grabbed from behind by another man with a large knife; he stabs the man in the neck, shoulder and leg (we hear a crunch and see blood as the man screams), and the attacker pushes his fingers into the other man’s bloody shoulder wound and pulls something out (tendon or bone) and snaps it as the man continues to scream.
► A man cuts a young woman’s face (we see blood and stitches later). A man cuts another man’s throat (blood gushes), and then stabs several others repeatedly (blood sprays and gushes and drips off the knife blade). A crimes scene shows several bodies covered with plastic tarps and one decapitated body tied in a bed (we see a lot of blood, bone and matter at the shoulder and neck) as another man drops the bloody severed head on a road as he drives (blood splatters on the road as the head rolls).
► A man punches a man in the face knocking him unconscious (we see blood on the attacker’s hands and the man’s face). A man slams an apartment door open, threatens a man inside and punches a wall. A man grabs a young woman’s hand and twists her arm; he slams a large knife into a table and startles her, and then pulls the knife out and takes her to a club. A man grabs a young woman by the hair and takes her through a dark hallway; she is held and threatened and the man injects her in the arm with something that causes her to fall unconscious.
► Explosives blow up under vehicles and one is thrown into the air in flames. A man walks through a tunnel and has a flashback to a war scene with gunfire and explosions ringing out, a flame thrower and large flames, and we see a soldier fall to the ground injured (no blood is evident). A man stands over a newly dug grave. An extended scene shows a man arming many weapons and booby-trapping tunnels, a barn and the surrounding land. An assortment of large guns, ammunition and knives are seen on a wall in a bunker. A man forges and hammers glowing red metal.
► A man with bloody wounds on his face is stitched up by another man (we see blood and heavy stitches in his face). A young woman’s arm bleeds from having been repeatedly injected with drugs and we later see numerous track marks. Young women being held by men and used for sex are shown with bruised and scabbed faces. We hear that a man has a concussion after a beating.
► A man yells at and threatens several young women and pushes one that tried to run away to her knees as she cries; the man says that if anyone else tries to run, he will kill them. A man yells at a young woman and tells her, “I’m gonna put a bullet in your head” (he does not). A man threatens another man and says, “I’m gonna reach in your chest and rip out your heart.”
► A woman yells at her adult niece about seeing her estranged father. A man says of another man, “He’s not a good man.” A young woman says that a man “freaked them out,” referring to friends of hers. A man yells at a young woman about another man having thrown his family away. A man tells a young woman that her father beat her mother with a belt. A man tells his adult daughter, “You didn’t mean anything to me,” and “I never wanted you.” A woman tells a man that her sister was kidnapped and found dead from an overdose. A man tells a woman that her niece is dead and she weeps mournfully. A woman says, “I will be sad until I die.”

Young men and women are shown holding and drinking from bottles and cups of beer, two men hold bottles of beer while in a parking lot and one man drinks from his bottle, and a young woman says that she drank too much one night when her friend disappeared. A pack of cigarettes and an ashtray filled with butts are seen on a table, and several people smoke cigarettes in a few scenes.
